Practical answers for community partners

Which families qualify?
People with a dementia diagnosis on Original Medicare Parts A and B, not in hospice or PACE, living at home. Medicare Advantage members don’t qualify for GUIDE.
How does the referral fee work?
A set fee for each family enrolled, in a written agreement reviewed by counsel on both sides.
Do families keep their choice?
Yes. Families always choose whether to enroll and can keep using any provider.
How much staff time does it take?
A referral is a short secure web form. Delivery partners plan staff time with us in week 1.
What can we report to our funders?
Families referred and enrolled, caregiver strain over time, respite hours delivered and families still connected to your services.
How is family data protected?
Shared under a signed data agreement, in a secure platform.
Do we need new payer contracts or IT?
No. Staff work in one web portal, and we handle eligibility, documentation and billing.
What if a family doesn’t qualify?
They stay with you. We’ll tell you quickly, and no fee applies.
